Nacho Soler wins the prize! He has identified almost all the faces in the Latin America Mystery Photo. He identified the leaders of Bolivia, Venezuela, Honduras, and Nicaragua--with only that pesky white-shirt guy continuing to stump both him and JE.
It must also be said that the guess about the moment was close but not right on. When the photo was taken, Zelaya had already been president of Honduras for a couple of years. Instead, this was the moment in 2008 when Mel Zelaya went whole hog for the pink tide, and joined the Chávez-Castro alternative trading pact, ALBA, alongside Evo Morales of Bolivia, Daniel Ortega of Nicaragua, and a plethora of small Caribbean island nations wanting bargains on Venezuelan oil. At the photo moment in 2008, the writing was already on the wall for Chávez's mortgaged Venezuela, but the recruitment of Honduras into the bloc could possibly have marked a strengthening at least in image. Instead, it was probably a major factor in the Honduran Supreme Court ousting Zelaya the next year, without directly saying he was getting to look too Chávez-like.
But still there's that mysterious guy in the white shirt, still unidentified. He stumped me, too, when I first stumbled into this current. Just goes to show how much we all only thought we knew about the hierarchy of that same locale where JE now moderates at poolside. Could there be another Cuban bigwig doctor who's not Che? Try Googling those hints from my original post together with "mieles" and "doctor." His trajectory is almost as mysterious as the embassy brainwaves.
JE comments: I believe I've Googled the answer. He's almost invariably dressed in white in photographs. Might we say, Gary, that the white-shirt guy is the Cuban equivalent of Mike Pence, if the latter were a physician, an octogenarian, and a hard-line revolutionary?

JE comments: Carlos Lage Dávila was once thought of as a successor to the Castros, but he was removed from office in 2009. Fidel accused him of succumbing to the "honeys of power." Wikipedia tells us that he (Lage) has returned to practicing medicine.
